The Vita initiative, a partnership of CHARTER OAK COMMUNITIES, STAMFORD HOSPITAL and the CITY OF STAMFORD, is a forward thinking effort to address the social determinants of health in Stamford’s West Side. Arising from a community-engaged strategic plan, Vita has sponsored development of new mixed-income housing communities, a volunteer-powered urban farm, among other innovative programs, to create a health-focused neighborhood. The power of collaboration, and the recognition that complex social challenges require multi-sector solutions, has led to the establishment of the Social Innovation Platform and Dovetail to support sustainable, holistic solutions and greater social impact.
